Introduction: Odontogenic malformations of teeth can be frequently encountered affecting both the primary and permanent dentition. The co-occurrence talon cusp in a fused tooth, however, is an uncommon developmental dental aberration that necessitates variety prophylactic procedures to minimize potential consequences. Often, presence talon's result pulpal necrosis due its deep extension, which increases functional complexity. Methods: present case describes fusion supplemental tooth maxillary conjunction with multiple cusps on labial palatal aspects extremely rare entity first kind. Results: Endodontic management followed by esthetic reconstruction affected considered as standard treatment cases involvement any odontogenically malformed tooth. Our also demonstrates establishment accurate diagnosis using three-dimensional imaging modality overcoming challenges periapical radiographs. Conclusion: In addition diagnosis, practitioners should have in-depth understanding origin, prevalence, clinical radiographic characteristics odontogenic malformations, AIDS effective strategy.
